What You’ll Do:

  • Complete an initial, comprehensive and ongoing assessments to determine home health and/or hospice needs.  
  • Develop patient focused care plans that establish goals, based on nursing diagnosis and incorporates palliative nursing actions.
  • Provide respectful, compassionate care in patients’ homes.
  • Act as Case Manager and assume responsibility to coordinate patient care for an assigned caseload.
  • Collaborate with interdisciplinary team members.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
