Job Responsibilities:

  • Provide admission, evaluation and follow-up skilled nursing visits for home health patients
  • Provide quality hands-on care, management and teaching of patient’s care plan in accordance with physician orders, under Clinical Manager's supervision
  • Coordinate appropriate care, involving all the other healthcare team members (such as Physical
  • Therapists, Occupational Therapists, Home Health Aides and Medical Social Workers)
  • Report patient’s progress to physician and Clinical Manager as required to maintain continuity of care
  • Implement plan of care with patient and family to assist them in achieving optimal outcomes

Qualifications:

  • Valid RN License to practice in CA
  • Current BLS/CPR certification
  • Valid CA Driver’s license
  • Minimum of one year nursing experience is preferred
  • Med Surg, ICU, ER, Acute Experience, Home Health experience is a plus
