Quality Assurance Nurse (RN or LPN)
Location: 2100 N Gemini Rd, Flagstaff, AZ
Schedule: Flexible (7am-3pm, 8am-4pm, or 8am-5pm), Monday-Friday, 8 hours/day. Some weekends may be required.
Position: 1 Full-Time
We are seeking a dedicated RN or LPN to join our team as a Quality Assurance Nurse, covering QA, Staff Education, and Infection Prevention.
Responsibilities:
  • Implement and monitor policies, procedures, and compliance with state, federal, and VA regulations.
  • Conduct audits, data analysis, and CASPER report reviews to develop and track Quality Assurance Performance Improvement Plans (PIPs).
  • Prepare and interpret reports, statistical analyses, and summaries for staff.
  • Assist in developing and implementing corrective action plans and monitor regulatory compliance.
  • Investigate incidents of non-compliance and allegations of abuse, neglect, or mistreatment.
  • Chair and coordinate Quality Assurance and Infection Control Committee meetings.
  • Identify staff training needs, develop educational programs, and track compliance.
  • Conduct daily infection control surveillance and map infections using McGeer criteria.
  • Stay updated on industry guidelines from CMS, CDC, and other relevant sources.

Knowledge & Skills:
  • Management and administration principles.
  • Quality assurance, utilization review, and nursing home regulations (including OBRA).
  • Statistical and data-gathering techniques.
  • Adult education and staff training strategies.
  • Strong verbal, written, analytical, and problem-solving skills.

Requirements:
  • Current Arizona RN or LPN license.
  • Infection Preventionist Certification.
  • DPS Fingerprint Clearance Card.
  • Valid Arizona Driver's License and ability to travel in-state.
