Job description

Position Overview

Oasis Home Health Care is seeking a compassionate, reliable, and skilled Registered Nurse (RN) to provide high-quality nursing care to patients in their homes. The RN will assess patient needs, develop and follow individualized plans of care, coordinate with physicians and interdisciplinary team members, and help patients maintain safety, independence, and dignity in the home setting.

Key Responsibilities

·         Provide skilled nursing visits in accordance with physician orders, the plan of care, and applicable state and federal regulations.

·         Complete comprehensive patient assessments, reassessments, medication reviews, and clinical documentation accurately and timely.

·         Develop, implement, and update individualized care plans in collaboration with patients, families, physicians, and the interdisciplinary care team.

·         Educate patients and caregivers on medications, disease management, safety precautions, infection prevention, and self-care techniques.

·         Monitor patient progress, identify changes in condition, and report concerns promptly to the physician and appropriate team members.

·         Coordinate care with home health aides, therapists, social workers, and other clinicians to support patient goals and continuity of care.

·         Participate in case conferences, quality improvement activities, and required agency trainings.

·         Maintain patient confidentiality and follow all HIPAA, agency, and professional nursing standards.

Qualifications

·         Current, unrestricted Registered Nurse license in the state of practice.

·         Minimum of one year of nursing experience required; home health experience preferred.

·         Current CPR certification required.

·         Strong clinical assessment, communication, documentation, and time-management skills.

·         Ability to work independently while maintaining close communication with the care team.

·         Reliable transportation, valid driver’s license, and current auto insurance required for home visits.

·         Ability to pass required background checks, employment verification, and any applicable fingerprint clearance requirements.

·         Experience with OASIS documentation and electronic medical records preferred.

Schedule and Work Environment

This position involves travel to patient homes within the assigned service area. Schedule may include full-time, part-time, PRN, weekday, weekend, and/or on-call availability depending on agency needs. The RN must be comfortable providing care in diverse home environments and prioritizing patient safety during each visit.
