As a valued member of our team, we are seeking a highly skilled and dedicated Registered Nurse Assessment Coordinator (RNAC) to join our team in Dover, DE.
About the Role:
As a Registered Nurse Assessment Coordinator (RNAC), you will be responsible for providing accurate and timely MDS assessments in accordance with state and federal regulations. Your primary focus will be on ensuring the interdisciplinary team completes designated sections of the MDS assessment, and the MDS is coded accurately with information obtained via medical record review and completed in a timely manner.
Responsibilities:
  • Provide accurate and timely MDS assessments in accordance with state and federal regulations
  • Coordinate the RAI process, ensuring compliance with Medicare, Medicaid, OBRA, and other payor program requirements
  • Collaborate with the healthcare team to ensure seamless communication and coordination of patient care

Qualifications:
  • Current RN license valid in the state of Delaware
  • Experience as an MDS Coordinator
