Location: Hybrid - Wellspring Hospice | Frankenmuth
Service Area: Saginaw, Frankenmuth, Bay City & surrounding communities
Position Type: Full-Time OR Per Diem
Pay Range: $39.00 - $43.00 / hourly

What You'll Do
  • Provide compassionate, patient-centered hospice care in homes and facilities
  • Complete initial and ongoing patient assessments
  • Develop, implement, and adjust individualized plans of care
  • Manage pain and symptoms to ensure comfort and dignity
  • Administer medications and treatments as prescribed
  • Support and educate patients and families throughout the care journey
  • Collaborate with physicians and the interdisciplinary hospice team
  • Participate in team meetings to coordinate care
  • Maintain accurate and timely clinical documentation
  • Supervise hospice aides (bi-weekly visits)
  • Travel daily within the service area
What You Bring
  • Current Michigan Registered Nurse (RN) license (required)
  • Graduate of an accredited nursing program
  • 1+ year of nursing experience (required)
  • Additional experience in home health, med-surg, critical care, or long-term care preferred
  • Strong clinical judgment and communication skills
  • A compassionate, dependable approach to patient and family care
  • Ability and willingness to travel (mileage reimbursed)
Schedule & Compensation
  • Schedule: Full-Time ORPer Diem
  • Day Shift (weekday schedule)
  • On-Call Rotation: 1 evening after-hours per week & 1 weekend every 4-5 weeks
  • Pay: $39.00 - $43.00 / hour (based on experience & licensure)
