What You'll Do as an RN Case Manager:
As a Registered Nurse Case Manager (RNCM), you will coordinate and oversee the care of patients in their homes while serving as an advocate for their health and well-being.
  • Conduct in-person patient interviews and comprehensive nursing assessments
  • Develop, implement, and continually evaluate individualized patient plans of care
  • Oversee and coordinate care provided by HHAs, LPNs, RNs, and other caregivers
  • Communicate patient conditions and collaborate with physicians and other healthcare providers as patient needs change
  • Educate patients and families on home health care procedures, including wound care, IV administration, and medication management
  • Help reduce hospital readmissions through preventative care and continuity of care
  • Monitor patient progress and make appropriate updates to the plan of care
What You'll Need:
  • Current, unrestricted RN license in the state associated with this position
  • Current CPR/AED/BLS/First Aid certification
  • Reliable transportation to and from patient care sites
  • At least 1 year of professional RN experience in home health or a similar setting
  • Previous case management or utilization review experience preferred
  • OASIS experience preferred
  • Practical tracheostomy and/or ventilator experience preferred, but not required
