Overview

We are looking for a Full-Time Registered Nurse (RN) for a Hospice Clinical Care Coordinator position in the home hospice setting. This role is often referred to as the Director of Clinical Services in many hospice agencies.

The Clinical Care Coordinator is responsible for the overall direction and oversight of hospice clinical services. This leadership position helps establish, implement, and evaluate goals and objectives that support high-quality patient care while ensuring compliance with Conditions of Participation, State regulations, and company policies and procedures.
