What you'll bring:
- RN
- One of the following educational backgrounds and experience:
- A Bachelor's degree in Social Work, Human Services, or a related field.
- A Bachelor's degree in any field, plus two years of human services experience.
- An Associate's degree in Behavioral Science, Social Science, or a closely related field, plus two years of human services experience.
- Three years of human services-related experience.
Relevant fields of study often include: Rehabilitation, Nursing, Counseling, Education, Gerontology, Human Services, and Sociology.
Valuable experience for this role includes:
- Case management or related human services experience.
- Paid professional experience with aging and/or disability populations or programs (e.g., as a case manager, rehabilitation specialist, health specialist, or social services coordinator).
- Assessment and care planning experience with clients.
- Direct experience working with individuals with disabilities or mental illness.
- Assisting individuals and groups facing economic disadvantage, employment challenges, abuse/neglect, substance abuse, aging, disabilities, or inadequate housing.
- Experience in prevention, health, or cultural competencies.

Requirements:
Requirements to apply (one of these is essential):
- RN or LPN license.
- Bachelor's degree in Social Work, Human Services, or a related field.
- Bachelor's degree in any field with 2 years of Human Services experience.
- Associate's degree in behavioral science, social science, or a closely related field, plus 2 years of Human Services-related experience.
- 3 years of demonstrated Human Services-related experience (examples include CMA, CNA, Case Management, Home Health, and Personal Care Assistant).
Beyond your qualifications, you'll need:
- Strong communication and organizational abilities.
- The capacity to manage multiple tasks effectively.
- Flexibility for 50-75% travel.
- The ability to pass all required background checks and drug screenings.
Prior experience with the Home and Community Based Waiver Program is a plus, but it's not a strict requirement. We'll discuss compensation during your interview.
