Home Health Nurse (RN Case Manager)
$5,000 SIGN-ON BONUS FOR FULL-TIME RN CASE MANAGERS
Care IV Home Health is a locally owned and operated home health agency in Hot Springs seeking a full-time RN Case Manager to join our esteemed team. As an RN Case Manager with Care IV, you will enjoy a professional and supportive environment that fosters flexibility, work-life balance, and meaningful patient relationships. Requirements:
  • Registered Nurses (RN) license that is active and in good standing with the Arkansas State Board of Nursing.
  • Experience with home health nursing and OASIS is preferred; however, we will consider candidates with ICU, ER, med/surg, or related experience.
  • A minimum of three professional references on the application or upon initial contact.

Duties may include, but are not necessarily limited to:
  • patient education
  • medication administration
  • wound care
  • post-surgical care
  • skilled nursing care / direct patient care
  • patient follow-up
  • OASIS Start of Care (SOC), regular SOC, admits & discharges
  • Additional duties as assigned
