• Act as a liaison between clients and their families, as well as outpatient healthcare providers, to ensure seamless communication and continuity of care.
• Engage with insurance panels to manage pre-certification and utilization effectively, advocating for client needs.
• Participate actively in discharge and aftercare planning, guiding clients through transitions and ensuring they have the resources necessary for sustained well-being.
• Provide therapeutic meal support to clients, modeling healthy relationships with food and assisting in developing coping strategies.
• Deliver empathetic, affirming care that respects each client's unique circumstances and intersecting identities, fostering a safe therapeutic environment.

Your role will entail working with a diverse clientele, including older adults, who often face unique challenges related to mental health. You will leverage your expertise in this demographic to address their specific needs and craft individualized treatment plans that resonate with their life experiences and contexts.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of California is mandatory.
• Experience working with older adults or within a community health setting is essential to ensure effective rapport and integrity in your practice.
• Strong clinical and diagnostic skills are necessary for accurate assessments and treatment planning.
• Exceptional communication and interpersonal skills to build successful relationships with clients and their families.

• Experience in utilizing electronic health record (EHR) systems for managing client information and progress notes.
