• Conduct thorough assessments and develop individualized treatment plans tailored to the unique needs of each client.
• Facilitate consultations for creating pre-crisis and crisis management plans, behavioral guidelines, and medication treatment plans.
• Complete all required documentation accurately and promptly, ensuring compliance with state regulations and clinic standards.
• Utilize evidence-based screening tools for client evaluations as prescribed by the clinical team.
• Collaborate across departments to design and implement positive behavior support plans that improve client outcomes.
• Engage in licensing and accreditation processes to maintain the highest standards of service delivery.
• Act as a liaison to state agencies and community partners, fostering relationships that enhance client care and access to resources.
• Maintain open lines of communication with all stakeholders to uphold the clinic's mission and standards.
• Represent the clinic in community meetings, workshops, and professional development events to establish the clinic as a trusted resource in the mental health field.
• Perform additional duties as assigned by the Clinic Director to support the overall mission of South Coast Medical Clinic.
