Job description

Pay Range:

$52,395.20 - $92,164.80Position Summary

Provides disease specific management, a working knowledge of oncology principles, the ability to manage chemotherapy and the side effects, and can monitor and address symptoms. Works with patients and families throughout the course of treatment and beyond to provide an understanding of their disease, their treatment plans, and address patient clinical and psychosocial issues as required. Symptom management calls are made proactively to anticipate needs of patients undergoing treatment and to manage side effects of disease and/or treatment.

The Care Coordinator documents assessment and interventions in real time in the electronic medical record. Will collaborate with the oncologist and clinic staff to assist in managing and coordinating patient care. The RN Patient Care Manager will support the mission of the Company and the Care Management department.

Key Performance Areas

KPA 1 - Product Knowledge

  • Maintains knowledge of value-based programs.

  • Provide ongoing education to all care managed patients.

  • Utilize and provide resources to answer to assist with patient questions and needs.

  • Effectively and efficiently triages patient medical issues telephonically. Utilizes interventions appropriately and communicates with the physician/clinic staff as necessary.

KPA 2 - Resources

  • Utilizes and distributes available resources to patients and care givers.

  • Obtain needed community resources utilizing the tools provided by management.

  • Follow patients throughout the care continuum and act as a liaison for the patient and caregiver.

  • Educates patients and families about Care Management services. Ensure patient has all information to make informed decisions regarding treatment, understands the role of Care Management, and how to access services appropriately. Documents timely and thoroughly in the electronic medical record.

KPA 3 - Compliance and SOPS

  • Maintains Standard Operating Procedures of Compliance ensuring requirements are communicated and managed according to accreditation standards, HIPAA, local, state and federal pharmacy regulations.

  • Complies with CMS regulations and follows Care Management Standard Operating Procedures.

  • Keeps Manager promptly and fully informed of any problems or unusual matters of significance.

  • Always maintain and ensure the confidentiality of all patient and employee information.

  • Ensure adherence to legal and company policies and procedures and undertake disciplinary actions if the need arises

KPA 4 - Communication and Customer Service

  • Ensure effective communication and premier customer service with internal and external customers.

  • Professionally answers all incoming phone lines, accurately navigates the phone system and transfer calls appropriately.

  • Professionally communicates to staff and other departments using internal instant messaging system, email or via phone.

  • Identify when additional information is needed and reach out to the appropriate department to obtain that information in a professional manner as needed.

  • Receive patient complaints and triage/resolve problems of a clinical nature and inform the Supervisor of all communication.

KPA 5 - Navigation and Documentation

  • Utilizes approved documentation standards.

  • Appropriately document patient communications in OncoEMR.

  • Go through patient EMR chart to update demographic information as needed while maintaining HIPPA guidelines.

  • Be able to identify the appropriate actions for a patient based on EMR documentation.

  • Use real time for all documentation and communication to supervisor.


Position Qualifications


Education

  • A graduate of an accredited RN Nursing program required.


Minimum Relevant Experience

  • Minimum of 5 years of overall nursing experience required.

  • Minimum of two (2) years of oncology experience required.

Certifications/Licenses

  • Active RN license in state of employment

  • OCN certification preferred.

  • Current CPR certification required.

Standard Workdays/Hours: M-F commonly 8:00am - 5:00pm. Position will be covering our Hawaii clinics and will need to work Hawaii business hours that may vary based on business need.
