• Conduct comprehensive and ongoing assessments to determine the unique needs of each patient, ensuring tailored treatment plans.
• Facilitate communication between supervisors, team members, families, and support systems to discuss progress and adapt care strategies as necessary.
• Establish and provide a detailed appointment schedule, outlining home visits and necessary observations to monitor client progress effectively.
• Organize and coordinate in-home visits as per the requirements of individual care plans, ensuring all documentation and chart reviews are thorough and compliant with legal standards.
• Implement and monitor the effectiveness of program plans, making adjustments as required for optimal outcomes.
• Participate actively in interdisciplinary team meetings to discuss client care, share insights, and enhance collaborative strategies.
• Prepare monthly and quarterly progress notes that provide clear documentation of client developments and treatment impacts.
• Lead training sessions for new and existing staff to ensure competency in the program's methodologies and documentation standards.

• Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.
• Must possess active and unrestricted licensure as a Licensed Professional Counselor in the state of Texas.
