Job description

Position Overview

Health Access Network is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. In this pivotal role, you will leverage your clinical expertise to address the complex needs of vulnerable populations, guiding them through challenges related to mental health, social circumstances, and resource access.

About the Organization

At Health Access Network, we are passionate about improving the lives of individuals and families within our community in Hempstead, New York. Our mission is rooted in a commitment to social justice and advocacy for those who often find themselves marginalized or underserved. We specialize in proactive community outreach, offering a spectrum of servicesโ€”including counseling, case management, and crisis interventionโ€”designed to empower individuals and forge resilience in the face of adversity.

Work Environment

This position offers the unique opportunity to work remotely, allowing for flexibility and the ability to focus on providing support to those in need without the constraints of a traditional office environment. As a member of our community health clinic, you will connect with colleagues and community partners virtually while making a tangible impact on individual lives through our comprehensive social services.

Key Responsibilities
  • Conduct thorough evaluations and assessments to identify the social, emotional, and environmental barriers affecting clientsโ€™ well-being.
  • Develop tailored service plans that encapsulate clientsโ€™ goals, proposed interventions, and methods for monitoring progress toward those goals.
  • Engage in individual and family counseling sessions, providing emotional support and therapeutic techniques to help clients navigate lifeโ€™s obstacles.
  • Facilitate crisis interventions and risk assessments, ensuring that clients have personalized safety plans and adequate support during emergencies.
  • Establish collaborative relationships with community organizations to coordinate comprehensive care and advocate effectively for clients in need.
  • Participate actively in community outreach to raise awareness about our services and promote engagement with the clinic.
  • Engage in ongoing professional development through participation in team meetings, case conferences, and staff training to enhance the quality of care provided.
Patient Population / Client Focus

Your work will have a direct impact on a multifaceted client base, including individuals and families facing challenges related to homelessness, domestic violence, mental health disorders, substance abuse, and poverty. By employing a strengths-based approach, you will advocate for clients while actively listening to their stories, validating their experiences, and supporting their journey toward positive change.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited program is required.
  • Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Strong understanding of social and economic justice issues affecting the community.
  • Proficiency in trauma-informed care principles and culturally competent practices.
  • Exceptional communication, interpersonal, and problem-solving skills.
Preferred Experience
  • Previous experience within a social service agency or healthcare setting is preferred, particularly in roles involving case management or counseling.
  • Familiarity with local community resources and service providers that can enhance client care.
Compensation and Benefits

The annual salary for this position ranges from USD 60,000 to 100,000, reflective of experience and qualifications. In addition to competitive compensation, Health Access Network offers a comprehensive benefits package, including:

  • Medical, dental, and vision health insurance coverage to support the well-being of you and your family.
  • Retirement plans with employer matching contributions, helping employees secure their future.
  • A generous paid time off policy encompassing vacation, personal days, and holidays.
  • Opportunities for continuing education and training to further professional development and skills.
  • Insurance benefits to enhance your overall health and security.
Career Growth Opportunities

At Health Access Network, we are committed to fostering the professional growth of our team members. You will have access to training and mentorship opportunities designed to equip you with the skills necessary for advancement within the organization. We encourage continuous learning and offer pathways for career progression in various specialized areas of social work.

Why Join Our Team
