Position Overview

Strong Minds Counseling Center is actively seeking a dedicated and skilled Licensed Clinical Social Worker (LCSW) to join our remote team, focusing on hospice care. In this full-time role, you will play a critical part in helping individuals and families navigate the complexities of mental health and psychosocial challenges during some of the most vulnerable times in their lives. Key Responsibilities
  • Conduct thorough assessments and evaluations of clients to identify their mental health needs, social determinants, and treatment goals through a structured intake process that includes dialogue, psychosocial histories, and family interviews.
  • Provide one-on-one counseling sessions utilizing evidence-based therapeutic modalities to address a wide range of mental health and psychosocial issues, including anxiety, depression, grief, and adjustment disorders.
  • Facilitate group therapy sessions, creating a safe environment where clients can share experiences and support one another while receiving psychoeducation on common mental health issues.
  • Develop and implement personalized treatment plans that align with clientsโ€™ goals, including specific, measurable objectives and interventions tailored to their unique situations.
  • Provide crisis intervention services when necessary, including conducting risk assessments and safety planning, and coordinating emergency resources such as mobile crisis teams or hospitalization when needed.
  • Advocate for clientsโ€™ rights, ensuring they have access to healthcare and community resources, empowering them to navigate complex systems effectively.
  • Maintain accurate and confidential client records, ensuring that all documentation, including assessments, treatment plans, and progress notes, complies with legal and ethical standards.
Patient Population / Client Focus

Your primary focus will be on clients receiving hospice care, which encompasses not only the terminally ill but also their families navigating emotional and logistical challenges during the end-of-life process. You will provide support that is sensitive to cultural and individual differences, recognizing the diversity of beliefs surrounding death and dying.

Qualifications
  • A Masterโ€™s degree in Social Work (MSW) from an accredited institution with a strong foundation in clinical practice.
  • State licensure as a Licensed Clinical Social Worker (LCSW) is essential for this role.
  • Demonstrated clinical experience in providing social work and counseling services, preferably within hospice or palliative care settings.
  • Strong interpersonal and communication skills, with the ability to foster trusting relationships with clients and families.
