Key Responsibilities
  • Establish therapeutic relationships with patients while addressing resistance and conflict effectively. This includes working with patients, families, healthcare team members, and community representatives to foster open communication and trust.
  • Maintain comprehensive, up-to-date patient records adhering to approved documentation standards. All patient assessments, treatment plans, interventions, family involvement notes, and case closures must meet social work and patient care area standards.
  • Demonstrate proficiency in the latest DSM diagnostic criteria, particularly for children and adolescents, and apply this knowledge in understanding human growth and development.
  • Utilize behavior modification principles, assertiveness training, and supportive therapy methods to implement effective treatment strategies tailored to individual patient needs.
  • Conduct thorough psychosocial assessments by gathering relevant data through interviews, consulting with healthcare providers, and reviewing patient charts to craft appropriate treatment plans.
  • Develop, revise, and implement personalized treatment plans based on continuous psychosocial assessments that reflect patientsโ€™ cultural values and personal beliefs.
  • Facilitate educational and therapeutic group sessions, ensuring clear documentation of group dynamics and individual patient outcomes.
  • Participate in treatment-planning meetings to provide updates on patient progress, collaborate with colleagues on treatment strategies, and document observable changes in patient treatment plans.
Patient Population / Client Focus

Your focus will primarily be on individuals undergoing detoxification from substance use. This patient population often experiences a wide range of emotional and psychological challenges, requiring a supportive and empathetic approach. By understanding their unique circumstances and cultural backgrounds, you will help patients and families navigate their treatment journeys effectively.

Qualifications
  • A Master's degree in Social Work, Counseling, Psychology, or a related mental health discipline.
  • Hold an independent License as a Licensed Clinical Social Worker (LCSW).
  • Highly organized with the ability to manage multiple cases and documentation efficiently.
  • Strong communication and interpersonal skills for effective interaction with clients and team members.
  • Competence in interpreting diverse forms of data and instructions.
  • Basic Life Support (BLS) and CPR certified.
Preferred Experience
  • Demonstrated experience working with patients in detox or substance abuse treatment settings.
  • Experience in providing therapeutic group sessions and educational workshops.
