Key Responsibilities
  • Perform comprehensive assessments and diagnoses of patients, as necessary, using tools such as the Diagnostic and Statistical Manual of Mental Disorders to inform personalized care plans.
  • Continuously monitor and evaluate patient progress, implementing effective interventions and support strategies that align with individual mental health diagnoses.
  • Prepare and maintain thorough treatment records, progress notes, and other clinical documentation in compliance with regulatory standards and organizational policies, leveraging current technology for efficiency.
  • Engage collaboratively with patients and their families to develop and refine comprehensive treatment plans that incorporate desired outcomes and are tailored to the specific needs of each individual.
  • Contribute to patient safety and symptom reduction efforts by formulating treatment objectives alongside patients and their families.
  • Act as a bridge between patients and external treatment resources by coordinating referrals and establishing relationships with other clinicians and referral agencies.
  • Mentor students, interns, and other clinical staff in best practices and effective patient engagement strategies, enhancing professional development across the team.
  • Support team dynamics by participating in peer reviews, clinical discussions, and quality improvement initiatives directed by clinical supervisors.
Patient Population / Client Focus

Qualifications
  • Master's degree in Social Work from an accredited institution.
