• Conduct thorough patient intakes that focus on diagnostic assessments and functional evaluations, subsequently developing personalized therapy objectives and treatment plans.
• Provide crisis intervention and immediate mental health support to clients experiencing acute distress, ensuring they receive timely care.
• Collaborate actively with primary care teams to manage mental health issues, helping to enhance the overall effectiveness of patient care services.
• Perform regular follow-up evaluations to assess treatment effectiveness, monitoring patient progress, and adjusting therapeutic strategies as needed.
• Help patients develop self-management skills and relapse prevention strategies that empower them in their recovery journeys.
• Maintain accurate and detailed records of patient interactions, progress notes, and treatment plans in compliance with Agency and Federally Qualified Health Center protocols.
• Offer guidance to primary care practitioners on referrals for specialized mental health services, ensuring patients receive the most appropriate care.
• Actively participate in collaborative treatment planning with healthcare colleagues, sharing insights and recommendations for the best patient outcomes.
• Advise on community resources and program development that can aid in addressing the personal and social implications of mental health issues.
• Maintain adherence to the Clinical Professionals' Code of Ethics and engage in ongoing professional development opportunities.

You will work with a diverse patient population that includes individuals of varying ages, backgrounds, and mental health conditions. Our clients often face intricate social and psychological challenges, and it will be your responsibility to provide culturally sensitive care, employing evidence-based methodologies with a focus on empowerment and patient autonomy.

• A Master’s Degree in Clinical Counseling or a relevant field is mandatory.
• A valid Licensed Professional Counselor (LPC) license is required to practice.
• Strong knowledge of mental health evaluation, treatment planning, and a variety of therapeutic methodologies.
• Fluency in both English and Spanish is highly preferred to serve our bilingual community effectively.
• Demonstrated understanding of community mental health resources and their application in various treatment contexts.
• Ability to assess individual patient needs and tailor interventions accordingly.

• Experience working in community health settings, especially within Federally Qualified Health Centers.
• A strong background in working with diverse populations, particularly those with limited access to mental health services.
• Familiarity with teletherapy and digital healthcare tools is an advantageous asset in providing remote services.

The salary range for this position is set between USD 90,000 and 105,000 annually, commensurate with experience and qualifications.
