Your responsibilities in this role will encompass a variety of tasks aimed at improving patient care:

• Utilize electronic medical records (EMR) to accurately document and communicate patient progress, ensuring adherence to the social work treatment plan.
• Collaborate with an interdisciplinary team, identifying patient needs and strategizing effective care plans to advance overall treatment goals.
• Provide education and emotional support to patients and families, particularly regarding transition plans and available resources.
• Stay well-informed about government and private payer networks to facilitate seamless patient transitions to appropriate levels of care.
• Engage with community resources to coordinate comprehensive care tailored to individual patient requirements.
• Advocate for patients and families by promoting patient rights and facilitating access to protective services and necessary resources.
• Exhibit strong awareness of ethical decision-making in the delivery of care, providing proper advocacy for patient autonomy.
• Ensure reporting of any vulnerabilities or suspected abuse is conducted in accordance with legal requirements, safeguarding patient welfare.
• Participate in initiatives designed to prevent readmissions, contributing to the continuous improvement of care delivery processes.
• Join in transition conferences with patients and families to facilitate clear communication and support during critical care transitions.
• Employ tools and technology to identify patients at risk for readmission, implementing strategic interventions as needed.
• Complete clinical handoffs for at-risk patients, ensuring they receive necessary referrals for ongoing care.
• Engage in additional duties as assigned to support the needs of the organization and its clients.
• Provide emotional and informational support to patients and families regarding their identified financial or social needs.
