Key Responsibilities
  • Conduct comprehensive assessments of patients to understand their unique needs and develop tailored treatment plans.
  • Regularly communicate and collaborate with supervisors, direct care team members, families, and guardians to ensure an informed approach to each individualโ€™s progress.
  • Provide a structured schedule of appointments, home visits, and observational tasks for effective monitoring of client developments.
  • Coordinate and perform home visits, integrating observations, chart reviews, and staff interviews for holistic care delivery.
  • Implement treatment plans effectively and ensure that interventions remain aligned with established goals.
  • Participate in interdisciplinary team meetings to discuss patient progress, treatment strategies, and necessary adjustments.
  • Document monthly and quarterly progress notes to track patient improvements and facilitate communication among team members.
  • Deliver training sessions to educate staff and colleagues on the program plan and the necessary documentation procedures.
  • Conduct follow-up interviews, home visits, and observational reviews to ensure ongoing support for individuals' needs.
  • Maintain accurate and accessible records to foster transparency and information sharing among stakeholders.
