We prioritize confidentiality in all patient interactions and accept Medicaid, Medicare, and most private insurance, enabling us to serve a diverse population effectively.

Work Environment

This role is structured as a remote position, allowing for flexible scheduling as we accommodate patient appointments. Key Responsibilities

As a Licensed Professional Counselor, your responsibilities will include:

  • Conducting detailed patient intakes that focus on diagnostic assessments and functional evaluations to inform treatment plans.
  • Providing brief, focused interventions for patients requiring mental health services, including crisis management and ongoing support.
  • Collaborating with primary care providers to identify patients with mental health and psychosocial needs, offering recommendations for treatment goals and plans.
  • Developing and implementing relapse prevention strategies aimed at maintaining stable functioning among patients.
  • Monitoring patients' progress and making necessary adjustments to treatment strategies based on their responses and outcomes.
  • Documenting client interactions, assessments, and treatment plans in compliance with our agency's requirements and Federally Qualified Health Center (FQHC) standards.
  • Facilitating connections with external treatment providers for comprehensive care, with the patient's consent.
  • Engaging in continuous professional development and adhering to the Code of Ethical Conduct for Clinical Professionals.
  • Carrying out additional duties as assigned by clinic management to support operational needs.
Patient Population / Client Focus

Your primary focus will be working with a diverse patient population who present with various mental health issues ranging from anxiety and depression to complex psychosocial challenges. By utilizing effective therapeutic techniques and collaborating within a multidisciplinary team, you will aid clients in developing self-management skills and a deeper understanding of their mental health. This role is pivotal in the prevention of psychiatric crises and enhancement of patients' quality of life.

Qualifications

To be considered for this role, you must have:

  • A valid and current LPC (Licensed Professional Counselor) license.
  • Solid knowledge of mental health assessment, treatment planning, and psychotherapeutic techniques.
  • Experience with the DSM-IV diagnoses and familiarity with behavioral therapy methods.
  • A deep understanding of the social and psychological factors impacting health and wellness.
Preferred Experience

While not mandatory, preference will be given to candidates who possess:

  • Bi-lingual proficiency in English and Spanish, enhancing our ability to serve a multicultural community.
  • Previous experience working within community health settings or with diverse populations.
  • Familiarity with patient management software and electronic health records.
  • Active participation in community outreach programs focused on mental health awareness and education.
Compensation and Benefits

We offer a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications. Comprehensive benefits include health, dental, and vision insurance, paid time off, and a SIGN-ON BONUS for new hires.
