• Conduct thorough psychosocial assessments for patients and their families, identifying emotional, social, and environmental challenges as they relate to diagnoses, treatment plans, and life situations.
• Develop and implement individualized treatment plans based on comprehensive assessments, employing appropriate social work interventions tailored to the needs of each patient.
• Collaborate effectively with the broader health care team, encouraging patient and family involvement in decision-making processes to ensure a holistic approach to care.
• Facilitate discharge planning that includes coordination with consulting physicians and third-party payers to create a seamless transition for patients moving from inpatient or outpatient care.
• Provide educational support and counseling to patients and families regarding coping strategies related to illnesses and life transitions, enhancing their adaptation to new challenges.
• Utilize a range of therapeutic modalities, including crisis intervention and both short-term and long-term individual and group therapies, to support clients effectively.
• Serve as a resource for other healthcare professionals, imparting your expertise in psychosocial care and conducting educational sessions to improve staff understanding and delivery of care.
• Participate in on-call rotations to provide timely support during after-hours calls, ensuring patients receive the care they need at critical moments.
• Present at educational conferences, showcasing your expertise in psychosocial oncology and contributing to the broader knowledge of social work practices in this specialized field.
• Engage in community outreach through education and volunteering, working to enhance local services and resources for cancer survivors and other vulnerable populations.
• Support the professional development of peers by aiding in the orientation, supervision, and evaluation of clinical social workers within the oncology setting.

The LCSW will primarily focus on a diverse client base, including individuals struggling with chronic illnesses, mental health challenges, and the psychosocial impacts of cancer. • A Bachelor’s Degree in Social Work (BSW) is required; a Master's Degree in Social Work (MSW) is strongly preferred.
• Current Licensure as a Clinical Social Worker (LCSW) in Colorado.

• Demonstrated ability to work collaboratively in a multidisciplinary environment.
• Excellent communication skills and a commitment to compassionate patient care.

• Prior experience working in oncology or with patients facing chronic health issues.

• Experience in providing telehealth services and familiarity with online communication tools.

The salary range for this position is competitive, offering an annual compensation of USD 70,000 – 100,000 commensurate with experience and qualifications.
