Key Responsibilities
  • Evaluate patients using bio-psychosocial assessments and apply individual clinical social work skills to address a variety of psychosocial challenges, ensuring each patient's unique needs are met.

  • Collaborate with a multidisciplinary team to create comprehensive care plans that integrate evidence-based treatment modalities, with the aim of improving patient outcomes and ensuring continuity of care.
  • Document social work activities and treatment interventions in patient medical records in a timely and accurate manner, adhering to departmental standards for frequency and content.
  • Facilitate individual and group therapy sessions, employing tailored techniques to support patient engagement and progress.
  • Provide education and resources to patients and families, guiding them in understanding treatment recommendations to foster optimal health functioning.
  • Assess the advocacy needs of patients and their families, coordinating with community resources to enhance support systems and promote self-determination within the client system.
  • Stay informed on community resources and actively refer patients to services that align with their specific needs.
  • Participate in ongoing professional development activities to maintain and enhance your clinical expertise.
Patient Population / Client Focus

Your work will primarily focus on patients within our detox center, many of whom are grappling with substance use disorders and related mental health issues. You will be instrumental in creating a supportive therapeutic environment that encourages patients to engage in their recovery journey. Understanding the complexities of addiction and mental health, you will tailor your approaches to meet the diverse cultural and individualized needs of each patient.

Qualifications
  • A masterโ€™s degree in Social Work from an accredited institution and a current license from the State Board of Social Work Licensure, specifically at the Licensed Clinical Social Work (LCSW) level.
  • A minimum of one year of clinical experience, preferably within a healthcare or mental health setting, where you have developed a deep understanding of the unique needs of our patient population.
  • Demonstrated knowledge and proficiency in applying social work principles and methodologies in a therapeutic context, particularly addressing the challenges faced by those dealing with mental illness and substance use disorders.
Preferred Experience

Compensation and Benefits
