Full Time LPN

Mileage Reimbursement

Monday-Friday 8a-4pm

Locations covering: North Franklin County, Union County and Delaware County

PURPOSE

According to prescribed policies and procedures of the organization including all applicable federal, state and accreditation regulations and reporting to the Clinical Leader, the Licensed Practical Nurse is responsible for providing nursing care services and case management to clients and families by implementing nursing interventions set forth by the plan of care.

ESSENTIAL FUNCTIONS

  • Provides direct patient care as defined in the State Nurse Practice Act and as ordered by the physician.
  • Implements plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse, RN Case Manager and/or Director of Clinical Management.
  • Provides mentoring, coaching, and education to the direct care staff, under the supervision of the RN.
  • Uses equipment and supplies effectively and efficiently.
  • Participates in personal and professional growth and development.
  • Participates in on-call rotation.
  • Participates in interdisciplinary team meetings to coordinate the care of the client and family; exchange information and problem solve; and receive staff support and education.

EXPECTATIONS

  • Demonstrates and maintains a positive, professional manner, manages time efficiently and effectively, provides leadership as a role model to promote a team approach, collaborates and participates in policy creation and appropriate problem-solving methods.
  • Must adhere to all policies, procedures, terms and conditions set forth in the National Church Residences' Employee Information Guide (EIG), the Code of Conduct, as well as any facility handbook, including but not limited to, corporate compliance, drug free workplace, safe work practices, all federal, state, local regulations and laws.
  • With respect to Resident Rights, ensures all care is provided with respect and dignity for the residents/clients, their families and the staff. Reports all complaints made by residents/clients and/or families to the appropriate supervisors, reports all allegations of abuse, misappropriation of funds/resident property and/or any other corporate compliance, drug free workplace, safe work practices, all federal, state and local regulations and laws.
  • Completes all annual education requirements timely, including but not limited to Compliance, Privacy, Security and HIPAA training and exhibits behavior as set forth by the Code of Conduct in the performance of their duties.
  • Performs other duties as assigned.

JOB SPECIFICATION SHEET - Licensed Practical Nurse

Education: Degree in nursing field as a licensed practical nurse.

Experience: 12 months facility/agency experience in nursing as an LPN preferred.

Travel: Continuous, necessary on a daily basis. Must have a valid driver's license, automobile insurance and qualified to drive under the organization's motor vehicle check.

Mental: Must have good verbal and written communication, comprehension, computer and interpersonal skills. Must have the ability to speak, read, write and understand English.

Licensure: Must have a current LPN license in good standing in the state(s) in which practicing.
