Position Overview

Polaris Rehabilitation & Care Center is seeking a compassionate, dependable, and clinically skilled Registered Nurse (RN) to join our nursing team.

As an RN, you will provide and oversee direct resident care while serving as a clinical leader and resource for nursing staff during your assigned shift. You will be responsible for resident assessments, medication and treatment administration, clinical decision-making, staff supervision, and coordination of care.

This position is ideal for a nurse who values hands-on resident care while also having the opportunity to lead, mentor, and support a nursing team. You will work closely with physicians, providers, CNAs, families, and interdisciplinary team members to promote positive clinical outcomes and ensure residents receive safe, individualized, and compassionate care.

Key Responsibilities
  • Provide and oversee nursing care in accordance with individualized care plans, provider orders, facility policies, and established clinical standards

  • Complete nursing assessments and continuously monitor residents for changes in physical, cognitive, behavioral, or psychosocial condition

  • Recognize changes in condition and initiate appropriate nursing interventions and notifications

  • Safely prepare and administer medications, treatments, and therapeutic interventions within the RN scope of practice

  • Communicate significant changes in resident condition to physicians/providers, families, and appropriate members of the interdisciplinary team

  • Supervise, delegate, direct, and support CNAs and other nursing personnel during assigned shifts

  • Provide clinical guidance and coaching to nursing team members to promote safe and consistent care

  • Collaborate with physicians, pharmacy, therapy, dietary, social services, and other interdisciplinary team members to coordinate resident care

  • Participate in admissions, transfers, discharges, and other resident care transitions

  • Maintain accurate, complete, and timely documentation within the electronic medical record (EMR)

  • Ensure physician/provider orders are accurately implemented and appropriately followed through

  • Educate residents and families regarding medications, treatments, care plans, and health-related needs within appropriate scope

  • Respond promptly to urgent and emergency situations using sound nursing judgment and established clinical protocols

  • Participate in infection prevention, quality assurance, resident safety, and performance improvement initiatives

  • Protect resident rights, dignity, privacy, preferences, and confidentiality

  • Immediately report suspected abuse, neglect, exploitation, mistreatment, injuries of unknown origin, or other resident-safety concerns through established reporting procedures

  • Maintain compliance with facility policies, professional nursing standards, and applicable federal and Wyoming requirements

Qualifications
  • Graduate of an accredited nursing program

  • Current, active Registered Nurse (RN) license in good standing in the State of Wyoming

  • Current CPR or Basic Life Support (BLS) certification, as required

  • Previous experience in skilled nursing, long-term care, rehabilitation, post-acute care, or geriatric nursing preferred

  • Strong clinical assessment and nursing judgment skills

  • Ability to recognize and appropriately respond to changes in resident condition

  • Demonstrated ability to supervise, delegate, and provide clinical direction to nursing personnel

  • Strong organizational and time-management skills

  • Excellent written and verbal communication skills

  • Ability to effectively communicate with residents, families, physicians/providers, and interdisciplinary team members

  • Strong attention to detail and commitment to accurate clinical documentation

  • Ability to prioritize multiple clinical responsibilities in a fast-paced healthcare environment
