Telos Health Systems is seeking an experienced Advanced Practice Provider to serve as an in-house SNF-ist, providing general medicine care to geriatric residents in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs). What you'll do
โ—† Serve as the facility's primary internal medicine provider (SNF-ist).
โ—† Manage care for short-term rehab and long-term care residents.
โ—† Perform comprehensive health assessments, physical exams, and diagnoses.
โ—† Manage acute and chronic conditions following established protocols.
โ—† Prescribe medications and treatments as appropriate and within scope.
โ—† Conduct monthly stability visits for long-term residents.
โ—† Evaluate residents with changes in condition.
โ—† Participate in Advanced Care Planning.
โ—† Collaborate with interdisciplinary teams, medical directors, and attending physicians.
โ—† Complete accurate documentation and CPT coding.
โ—† Facilitate referrals to specialists and other healthcare providers.
What we're looking for
โ—† Active, unencumbered Physician Assistant (PA) or Nurse Practitioner (APRN/APN) license in the applicable state.
โ—† Current DEA registration.
โ—† Master's Degree from an accredited program
โ—† Strong communication and clinical leadership skills.
โ—† Ability to work effectively in a fast-paced environment.
โ—† Preferred: at least one year of experience in a Skilled Nursing Facility or long-term care setting.
โ—† Preferred: experience caring for geriatric populations.
Compensation amp; benefits
W-2 employment with pay-per-encounter, fee-for-service compensation and flexible scheduling. Comprehensive benefits including medical and prescription coverage (Blue Cross Blue Shield), dental and vision (UnitedHealthcare), a 401(k) through Nationwide, voluntary Aflac benefits, 15 days PTO, six paid holidays, occurrence-based malpractice coverage ($1M/$3M), a $1,000 annual CEU, DEA, and licensure allowance, mileage reimbursement, and company-provided equipment, including a Microsoft Surface Pro.
