Job description

The role of the STI nurseย is to provide prompt follow up to all STI cases identified by our Hepatitis C outreach team, and coordinate care for newly diagnosed clients. Also, to engage prevention clients in care and ensure follow-up visits are scheduled.

Hep C

  • Rochester pharmacy sends list of meds delivered for the week. RN adds to spreadsheet

  • RN reaches out to outpatients after first box delivered to confirm meds in hand and answer any questions

RN reaches out to patients after last box delivered to remind them to text when they take their last pill.

  • RN reaches out after last pill to congratulate patient and let them know the MA will call them in 12 weeks to set up TOC

  • RN adds reminder to calendar for Emily to call pt for TOCย 

  • RN looks over tracker to make sure all patients were ordered meds that need them and meds delivered

Locked in pharmacies:

  • Email from Rochester notifying team of a locked in patient

  • Rochester adds patient to Hepatitis C locked in pharmacies spreadsheet

  • RN makes sure that NP has rescripted med

  • RN reaches out to patient to make sure they have contacted or been contacted by locked in pharmacy as well as to confirm meds in hand.ย 

  • RN assists any patients having difficulty obtaining medication from locked in pharmacy, including calling the pharmacy to troubleshoot the issue, obtaining or helping patient obtain a copay card.ย 

  • RN fills out Hep C locked in pharmacies spreadsheet information

STI: (including HIV)

  • NP notifies RN of a positive STI by adding patient to STI tracker

  • RN notifies DOH

  • RN reaches out to patient to make sure they received treatment and to answer any questions. Lab review by NP already done.ย 

  • RN fills in information on tracker

HepCMyWay:ย 

  • RN checks TreatSpace for new form submissions

  • RN checks Epic to determine if current patient

  • RN reaches out to patient via requested method to set up a time for a call to discuss the process.

  • RN emails leadsย who send to the MA in that area. The MA then reaches out to the patient to schedule a time to meet.

  • Afterwards the regular Hep C flow is followed

Prep2Me:

  • RN checks TreatSpace for new form submissions

  • RN checks Epic to determine if current patient

  • RN reaches out to patient to explain the program and confirm pt is in OH, PA or WV

  • Create chart in Epic and obtain insurance information

  • RN schedules appointment with NP for one month out

  • RN sends out testing kit from molecular labs

  • Patient is added to Prep2Me Tracker

  • Prior to appointment RN will confirm that testing kit was returned and results are showing for telehealth visit.ย 

  • Prior to each 3 month follow up, RN will send out a new testing kit.

Chart Checks:

  • RN will do chart checks and keep a spreadsheet that Emily has access to

Miscellaneous:

  • RN will call pharmacies to troubleshoot issues

  • RN will assist patients in getting copay cards for meds

  • Answer patient texts and calls. Answer questions and coordinate care

  • Monthly meeting at Rochester Pharmacy

  • Miscellaneous requests from NPs or MAsย 

  • Ability to balance multiple projects, prioritize deadlines, and work independently.
  • Active and unrestricted RN license.
  • Epic experience preferred.
  • Experience with Microsoft Excel, Microsoft Word, as well as Electronic Medical Records.
  • Experience with patient centered care.
  • Punctuality and consistent attendance.
  • Communicate effectively with colleagues, professionals, and patients from a variety of demographic backgrounds and identities.
  • Technical skills with required office equipment.
  • Ensuring visit notes and documentation are up to date per patient encounter within a 48-hour period.
  • Non-judgmental approach to patient care.
  • Patience and great attention to detail.
  • Complete understanding of the healthcare privacy laws as outlined in HIPAA.
  • LGBTQIA and PWID cultural competency.
