Optum Home & Community Care, part of the Optum family of businesses, is creating something new in health care. We are uniting industry-leading solutions to build an integrated care model that holistically addresses an individual's physical, mental and social needs - helping patients access and navigate care anytime and anywhere. As a team member of our Senior Community Care (SCC) team, we work to provide care to patients in nursing homes, senior housing and assisted living settings. This life-changing work adds a layer of support to improve access to care. We're connecting care to create a seamless health journey for patients across settings. Join us to start Caring. Connecting. Growing together.

You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The RN is a flexible team member who works alongside nurse practitioners (NPs) and physician assistants (PAs).

Standard Hours:  Monday-Friday 8:00 a.m. to 5:00 p.m. EST

This is a field-based position traveling to facilities throughout Cuyahoga County (i.e., primarily Western Cleveland - Berea, North Olmstead, & Middleburg Heights, OH) & Medina County (i.e., Medina, OH).

Primary Responsibilities:

  • Collaborating with all members of the interdisciplinary care team around changes in condition and/or care coordination
  • Triage member needs to the appropriate care team member and level of care
  • Assist with transitions in care: outpatient and inpatient
  • Assist in assuring complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessments

What Makes an Optum Career Different?

  • Nurses are supported to practice at the peak of their license
  • We believe that better care for nurses equates to better care for patients
  • We influence change nationally while maintaining the culture and community of local care organizations
  • We grow talent from within. No matter where you want to go- geographically or professionally- you can do it here

Compensation & Benefits Highlights:

  • Guaranteed, competitive compensation model based on quality, not quantity, with bonus opportunities
  • Financial stability and support of a Fortune 5 Company
  • Robust retirement offerings including employer funded contributions and Employee Stock Purchase Plan (ESPP for UHG Stock)
  • Comprehensive benefits plan inclusive of medical, dental, vision, STD/LTD, CE & equipment allowances
  • Robust nursing learning and development programs

You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.

Required Qualifications:

  • Active unrestricted Registered Nurse license in your state of residence or ability to obtain within 60 days of hire
  • Certified in Basic Life Support
  • Driver's license and access to reliable transportation that will enable you to travel up to 100% to visit clients and/or patient sites within a designated area

Preferred Qualifications:

  • BSN degree
  • 1 years of experience working as a RN (long-term care setting preferred)
  • Experience in geriatric medicine, long-term care, senior living or home care setting

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $60,200 - $107,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
