Houston County Jail Georgia
Full Time Monday through Friday 8:00 AM to 4:30 PM

Southern Health Partners is looking for a Registered Nurse (RN) to join our healthcare team at the Houston County Jail in Georgia as a Chronic Care Nurse.
This is an excellent opportunity for an RN who enjoys building relationships with patients, managing ongoing health conditions, and helping individuals stay on track with their healthcare needs. You'll have the opportunity to focus on long term care planning and disease management while working a consistent Monday through Friday schedule.
What You'll Do
As the Chronic Care RN, you will:
  • Provide nursing care to patients with chronic and ongoing medical conditions
  • Assess patient health status and develop appropriate plans of care
  • Monitor chronic conditions and evaluate patient progress
  • Coordinate follow-up care and communicate changes in patient condition
  • Provide patient education related to medications, treatment plans, and healthy behaviors
  • Collaborate with providers and the correctional healthcare team
  • Maintain accurate and timely clinical documentation
  • Help ensure continuity and quality of care throughout the patient's stay
What You'll Need
  • Current Registered Nurse (RN) license in the state of Georgia
  • Current CPR certification
  • Correctional healthcare experience is a plus, but not required
  • Strong assessment, communication, and organizational skills
  • Ability to work independently while collaborating effectively with the healthcare team
