Schedule: Monday–Thursday | 10-hour shifts (4x10) or Tuesday-Friday (4x10)

CoreLife, in partnership with Novant Health, is seeking an experienced Nurse Practitioner (NP) to join our multidisciplinary care team.

As a Nurse Practitioner, you will serve as the clinical leader of the care team, working closely with dietitians, exercise specialists, and behavioral health clinicians to manage obesity and related chronic conditions.

Key Responsibilities – Nurse Practitioner (NP)

  • Provide comprehensive medical evaluation, diagnosis, and treatment for patients with obesity and related conditions
  • Develop and manage individualized treatment plans aligned with evidence-based care
  • Lead a multidisciplinary team including:
    • Registered Dietitians
    • Exercise Specialists
    • Behavioral Health Clinicians
  • Monitor patient progress and adjust care plans to optimize outcomes
  • Manage chronic conditions such as:
    • Obesity
    • Diabetes
    • Hypertension
    • Metabolic syndrome
  • Ensure timely and accurate documentation in Epic EHR
  • Support patient engagement through education, coaching, and behavioral strategies
  • Maintain productivity and quality standards set by CoreLife and Novant Health

Qualifications – Nurse Practitioner

Required:

  • Master’s Degree or Post-Master’s Certificate from an accredited Nurse Practitioner program
  • Active Nurse Practitioner license in the state of practice
  • Current DEA registration
  • Registration with State Medical Board
  • Strong clinical knowledge of primary care or chronic disease management

Preferred:

  • 1–3 years of experience in a physician practice or outpatient setting
  • Experience in:
    • Weight management
    • Endocrinology
    • Lifestyle medicine
