You will be part of a multidisciplinary team in a Hospice Center environment, where integrated care and evidence-based methodologies are fundamental in delivering optimal patient outcomes.

• Evaluate, diagnose, and treat a diverse range of psychosocial issues, employing individual clinical social work skills to support patients and their families in overcoming challenges related to mental or physical illness.
• Collaborate with the Social Work Manager and multidisciplinary team to create comprehensive care plans that address both medical and psychosocial needs, thereby enhancing treatment compliance and patient satisfaction.
• Conduct in-depth bio-psychosocial assessments to identify potential barriers to treatment and to facilitate smoother transitions to discharge.
• Document all social work activities diligently in patient medical records according to departmental standards, ensuring adherence to ethical guidelines and regulatory requirements.
• Provide educational resources and advocacy to assist patients and families in understanding and executing medical and social work recommendations.
• Participate in regular team meetings to discuss patient progress and coordinate care efforts effectively.
• Maintain current knowledge of available community resources to better support patient advocacy in their transitions to recovery.
• Pursue ongoing professional development through training related to specific clinical responsibilities and uphold high standards of care.

This position focuses on individuals dealing with complex mental health challenges, often within the context of a hospice or palliative care setting. Your work will involve engaging with diverse populations, particularly those requiring support throughout their end-of-life transitions or dealing with chronic conditions. This role emphasizes understanding the unique cultural and developmental factors impacting each patient's experience.

• Master’s Degree in Social Work from an accredited institution, with current licensure as a Licensed Clinical Social Worker (LCSW) in the State of New York.
• Demonstrated capacity to understand and respond to the varied needs of patients, particularly relating to the aging processes and cultural sensitivities.

• At least six months to one year of clinical experience, ideally in health or mental health settings, with a focus on populations relevant to our patient base.
• Prior work in hospice or palliative care environments is highly valued.

The salary range for this position is competitively set between USD 85,000 and 100,000 annually, commensurate with experience.
