We seek a Full-Time Licensed Practical Nurse at our Crisis Residential Homes
Location: Suffolk, VA
Responsibilities & Duties for Full-Time LPN:
  • Implement and maintain Individual Service Plans for consumers.
  • Conduct ongoing physical and mental health assessments.
  • Maintain accurate consumer records and monitor safety.
  • Supervise and oversee individual medical treatment programs.
  • Administer and deliver prescribed medications as needed.
  • Educate consumers on medication compliance and potential side effects.
  • Assess vital signs, height, weight, and monitor medication compliance.
  • Complete prior authorizations and assist with psychiatric medication needs.
  • Ensure compliance with agency policies and relevant local, state, and federal regulations.
Qualifications and Skills for Full-Time LPN:
  • Graduation from an accredited Licensed Practical Nurse Program.
  • Current, unrestricted LPN license in Virginia or from a Nurse Licensure Compact State.
  • Minimum of 1 year of LPN experience in a mental health program and/or residential setting.
  • Minimum of 1 year of LPN experience working with individuals with intellectual disabilities.
  • Valid driver's license with an acceptable DMV record.
  • Proficiency in Microsoft Office.
  • Preferred: Experience with electronic medical records and basic knowledge of Medicaid/Medicare regulations.
