• Conduct thorough mental health assessments to understand patients' mental health concerns, taking into account their medical conditions and individual circumstances.

• Develop and implement personalized integrated treatment plans which address both psychiatric and medical needs of patients, ensuring comprehensive care that promotes well-being.

• Facilitate both individual and group therapy sessions utilizing evidence-based therapeutic techniques that cater to the unique requirements of each patient.

• Collaborate closely with multidisciplinary medical providers to ensure coordinated care and optimal treatment outcomes, enhancing the overall health of our patient population.

• Provide crisis intervention strategies and support during mental health emergencies, delivering real-time assistance to those in immediate need.

The Licensed Mental Health Therapist will serve a diverse patient population, including individuals with co-occurring medical conditions such as autoimmune disorders, substance use disorders, and chronic pain. • A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is required.

• Current and valid licensure as an LCSW, LPCC, or LMFT in the state of California.

• Demonstrated experience in supporting the mental health needs of individuals with co-occurring medical conditions.

• Exceptional interpersonal and communication skills, allowing for effective interaction with patients and interdisciplinary teams.

• Proficiency in maintaining professional boundaries and adhering to ethical guidelines while ensuring patient confidentiality.

• Familiarity with telehealth technology and electronic health record systems to facilitate efficient patient care.

• A strong inclination to work collaboratively as part of an interdisciplinary team focused on enhancing patient outcomes.

• Prior experience in a psychiatric or behavioral health setting, especially involving telehealth services.

• A background in integrated care models that address the intersection of mental and physical health.

• Certifications or specialized training in therapeutic modalities relevant to the populations served.
