Description
Our full-time Registered Nurse (RN) with have a variety of duties that will help the company thrive and manage continuity of care. This candidate must maintain 32 productivity points weekly to qualify and must participate in the compensated on-call program. This RN will see all visits assigned within the scope of practice of their license which includes OASIS visits, skilled nursing visits, and supervised visits. They will also participate in the compensated perception of new hires. Administer skilled nursing care for patients of all ages in their place of residence and to coordinate care with the interdisciplinary team, patients, families, and referring agency. Assumes the responsibility for, and participates in, the coordination of care.
Job Responsibilities
  • Case management and supervised visits.
  • Perform skilled and comprehensive assessment of the patient during the initial admission.
  • Assess and chart observations of the patient's condition at each visit (vital signs.
  • Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing the healing progress).
  • Provide proper patient education for home health care procedures and strategies.
  • Responsible for the overall clinical record maintenance to include visit note (must be completed within 24 hours), comprehensive assessments, including OASIS (must be completed within 48 hours), medication profile, orders/plan submitted in a timely manner, etc.
  • Maintain confidentiality of patient PHI through HIPPA law.
  • Keep the patient comfortable through palliative care

Qualifications:
  • Current Texas State license as a Registered Nurse.
  • 2+ years experience as a RN in Home Health.
  • Current CPR certification.
  • Reliable transportation.
  • Texas Drivers License and Auto Liability Insurance.
  • Current TB Skin Test
  • EMR experience (Kinsser preferred).
