Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

You’ll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion.

What You’ll Do

Lead Patient-Centered Hospice Care

• Conduct comprehensive patient and family assessments

• Develop and manage individualized, comfort-focused care plans

• Deliver skilled nursing care with a focus on pain and symptom management

Serve as the Care Coordinator

• Act as case manager, coordinating care across the interdisciplinary team

• Collaborate with physicians, social workers, chaplains, and aides

• Monitor patient condition and adjust care plans to maintain comfort

Support Patients & Families

• Provide education, emotional support, and guidance throughout the journey

• Help families navigate complex decisions with empathy and clarity

• Build meaningful, trusting relationships in deeply personal moments

Drive Quality & Compliance

• Maintain accurate, timely documentation

• Participate in IDT meetings and care planning

• Support quality and compliance standards

What You Bring

• Active RN license (or compact eligibility)

• Graduate of an accredited nursing program

• Minimum 1 year of clinical nursing experience

• Strong clinical judgment and compassionate care approach

• Ability to manage a caseload independently

• Excellent communication skills in sensitive situations

• Basic EMR proficiency

• Current CPR certification

• Valid driver’s license and reliable transportation
