Clinical Liaison Full Time

We are seeking a dedicated and experienced Clinical Liaison to join our team at Woodlake at Tolland, a renowned skilled nursing facility in Tolland, Connecticut. As a Clinical Liaison, you will play a vital role in maintaining hospital partnerships, managing patient safety and outcomes, and managing patient cases. About the Role:

As a Clinical Liaison, you will blend your clinical and marketing skills to:

  • Manage all referrals, reviewing for clinical appropriateness

  • Ensure all referrals are being responded to promptly

  • Work with the Admission Director to establish patient eligibility

  • Provide bedside sales and frequent hospital rounding

  • Coordinate with the facility to ensure a smooth transition from the hospital

  • Build strong relationships with referral sources, and maintain these relationships
