The Forward Function Healthcare Center is seeking a dedicated Occupational Therapist to join our team at the Early Interventional Center in Coos Bay, Oregon. Key Responsibilities
  • Conduct thorough assessments of residentsโ€™ functional abilities and needs to develop personalized therapy plans that support their mobility and independence.
  • Implement one-on-one treatment sessions that may include activities of daily living (ADLs), fine motor skills exercises, and adaptive equipment training.
  • Regularly monitor residents' progress towards their therapy goals, modifying care plans as necessary based on ongoing evaluations and clinical outcomes.
  • Maintain accurate and comprehensive documentation in compliance with healthcare regulations and ensure adherence to best practices in therapy techniques.
  • Collaborate with other healthcare professionals to integrate occupational therapy strategies into comprehensive care plans and participate in team meetings.
Patient Population / Client Focus

In this role, you will primarily work with older adults residing in an assisted living setting, many of whom may be managing chronic conditions or recovering from surgeries. Qualifications
