Licensed Practical Nurse โ€“ Integrated Healthcare

Salary: $26.00โ€“$33.00/hour + Every other Friday paid off (Wellness Day)

Sign-On Bonus: LPNs are eligible for a $2,500 sign-on bonus with a 1-year service commitment. Bonuses are paid out in increments within the first 6 months of employment.


Schedule: Full-Time | Mondayโ€“Friday | 8:00 AMโ€“5:00 PM | Every other Friday off (paid wellness day)

Your Role: Licensed Practical Nurse โ€“ Integrated Healthcare

Reports to: RN Program Manager

Position Overview

Heritage Behavioral Health Center is seeking a full-time Licensed Practical Nurse (LPN) for our Integrated Health Clinic, which provides primary and psychiatric healthcare services to individuals with serious mental illness and chronic health conditions.
This position is full-time, 40 hours per week, Monday through Friday, 8:00 AMโ€“5:00 PM.


Core Responsibilities
  • Accurately obtain and document vital signs, including blood pressure and other key indicators
  • Administer prescribed medications (weekly or as ordered) and report any adverse reactions to medical staff
  • Monitor health status and ensure timely, accurate charting in medical records
  • Communicate changes in patient condition clearly to providers
  • Provide compassionate, respectful, and professional support to patients and families
  • Maintain readiness of clinical supplies, medications, and equipment
  • Coordinate with pharmacy partners to ensure timely, accurate medication refills and adjustments
  • Facilitate referrals and link patients to appropriate healthcare services
  • Assist with triage, assessment, and coordination of care for urgent health concerns by collaborating with healthcare professionals
