Job description

Job Title: Home Health Care Manager
Department: Care Management
Location: Hybrid- 224 Alexander Street Suite 180
Hours Per Week: 40 hours per week
Schedule: 08:00am-4:30pm
Position Summary:
To provide supportive Health Home care management, utilizing the process of care coordination, for persons who are identified as high users of services and as having problems accessing medical, mental health, social, psychosocial, educational, financial and other necessary services that have been identified in a written plan of care. Provide 24/7 oversight as needed.
Key Responsibilities:
• Responsible for the overall creation, management and coordination across all continuums, of the enrollee's care plan which will include medical/behavioral and social service needs and goals.
• All necessary documentation must be completed within established timeframes.
• Effectively perform and document progressive community outreach with assigned cases and move to enrolled status as appropriate.
• Caseload will include clients with a mid-acuity rating and voluntary assisted outpatient treatment (AOT) clients. Completes weekly reports in accordance with AOT guidelines.
• Performs other duties as assigned
Minimum Qualifications:
• If hired on or after November 1, 2018, Bachelor's Degree required
• Two years of experience preferred
• QHP preferred
• Strong analytical, computer, and communication skills
Required Licensure/Certification Skills:
• Valid NYS driver license as referenced in RRHS Fleet Program

PHYSICAL REQUIREMENTS:
S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
