About this Position

The Perinatal Nurse Navigator supports pregnant and postpartum patients through a hybrid model of virtual and in-person care coordination across OSU Medicine Virtual Care's rural hospital and clinic partners. This role focuses on improving maternal and perinatal outcomes through care coordination, postpartum engagement, high-risk referral management, and support of transitions of care from pregnancy through the postpartum period

The navigator works closely with obstetrics, maternal-fetal medicine, behavioral health, addiction medicine, primary care, and rural hospital teams. The role includes patient navigation, education, remote monitoring support, referral management, and protocol-based clinical follow-up visits under physician/APRN supervision.

May have hybrid work options included some travel. 

Scope & Supervision

This role practices within Registered Nurse scope under OSU Medicine Virtual Care policies, standing orders, and supervising physician/APP oversight. Clinical activities are protocol-driven and do not include independent diagnosing or prescribing.

The Perinatal Nurse Navigator functions as a care coordination and clinical support resource and does not replace the patient's established obstetric or primary care provider relationship. Medical decision-making and treatment plans remain under the direction of the supervising provider and local treating clinicians.

Required Qualifications
  • Vocational/Technical
    • Registered Nurse (RN) license - Oklahoma or compact privilege
    (degree must be conferred on or before agreed upon start date)
    • 3-5+ years maternal/perinatal experience (L&D, postpartum, high-risk OB, MFM, or perinatal case management)
    • Care coordination and patient education experience

Certifications, Registrations, and/or Licenses:

  • Active Registered Nurse (RN) license

Skills, Proficiencies, and/or Knowledge:

  • Ability to travel to rural partner sites within Oklahoma
  • Effective communication and collaboration skills
Preferred Qualifications
  • Telehealth or virtual care experience
  • Rural health experience
  • Maternal quality or population health program experience
  • Social determinants of health navigation experience

Certifications, Registrations, and/or Licenses:

  • RNC-OB or RNC-MNN certification preferred (or obtained within 12-18 months)
