Position Summary
The Nurse Practitioner (NP) providescomprehensive primary, preventive, and episodic healthcare services to patientsof all ages at Health Care Partners of South Carolina. Working collaborativelywith physicians and an interdisciplinary healthcare team, the NursePractitioner assesses, diagnoses, treats, and manages acute and chronic medicalconditions while promoting health, disease prevention, and patient education.As part of a Federally Qualified Health Center (FQHC), this position iscommitted to delivering high-quality, patient-centered care to underserved andmedically vulnerable populations.
Essential Duties andResponsibilities
• Obtain comprehensive patient histories, performphysical examinations, and develop appropriate diagnoses and treatment plans.
• Assess, diagnose, treat, evaluate, and manageacute and chronic medical conditions across the lifespan.
• Order, interpret, and evaluate laboratory tests,diagnostic imaging, and other diagnostic studies.
• Prescribe medications, therapies, and othertreatments in accordance with South Carolina law and organizational policies.
• Perform routine office procedures within theNurse Practitioner scope of practice.
• Provide preventive healthcare services,including annual wellness visits, immunizations, health screenings, and chronicdisease management.
• Educate patients and families regardingdiagnoses, medications, treatment plans, disease prevention, and healthylifestyle practices.
• Coordinate patient care with physicians,behavioral health providers, pharmacists, dental providers, specialists, andother members of the interdisciplinary care team.
• Refer patients to specialists and otherhealthcare providers as clinically appropriate and monitor follow-up care.
• Participate in quality improvement initiativesand support HRSA Uniform Data System (UDS) quality measures and organizationalperformance goals.
• Maintain accurate, timely, and completedocumentation in the Electronic Health Record (EHR).
• Maintain patient confidentiality and protecthealth information in accordance with HIPAA and organizational policies.
• Participate in medical staff meetings,continuing education, and organizational quality improvement activities.
• Perform other duties as assigned.
Education & Experience
• Master of Science in Nursing (MSN) or Doctor ofNursing Practice (DNP) from an accredited Nurse Practitioner program required.
• Current, unrestricted South Carolina AdvancedPractice Registered Nurse (APRN) license required.
• Current national certification as a Family NursePractitioner (FNP) through the American Academy of Nurse Practitioners (AANP)or the American Nurses Credentialing Center (ANCC) required.
• Current DEA registration and South Carolinacontrolled substance authority required.
• Current Basic Life Support (BLS) certificationrequired.
• Experience providing primary care in a familypractice, community health center, or Federally Qualified Health Center (FQHC)preferred.
• Experience caring for underserved or medicallyvulnerable populations preferred.
• Bilingual (English/Spanish) skills are preferredbut not required.
Knowledge, Skills, and Abilities
• Knowledge of primary care principles,evidence-based clinical practice, chronic disease management, and preventivemedicine.
• Knowledge of federal and state regulations,HIPAA requirements, and FQHC quality standards.
• Strong clinical assessment, diagnostic, andcritical thinking skills.
• Excellent communication and interpersonal skillswith a commitment to compassionate, culturally competent, patient-centeredcare.
• Ability to collaborate effectively withphysicians and interdisciplinary healthcare teams.
• Proficiency in Electronic Health Record (EHR)systems and Microsoft Office applications.
