As a Nurse Practitioner, you’ll contribute to our success in the following ways:

  • Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
  • Prevents unwanted hospitalizations by meeting patient and family needs in the home.
  • Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
  • Compliance with applicable laws and regulations with respect to Collaborative Agreements.
  • Completes accurate billing and coding activities for all patient encounters according to CMS guidelines and initiates reimbursement for medically necessary visits in accordance with applicable law and regulation.
  • Provides training and continuing education for staff.
  • Assists in development of clinical practice guidelines/standards in support of quality palliative care.
  • Adheres to the practice of confidentiality regarding patients, families, staff and the organization.
  • Collaborates/Communicates with Medical Director, attending physician, branch leadership and IDG.
  • Performs other duties/projects as assigned.

What is Required?

  • Masters Degree in Nursing and certification through a national Nurse Practitioner Licensing board.
  • 3+ years clinical nursing experience
  • 1+ year post-graduate clinical nursing experience in the hospice and palliative care field.
  • Prior experience in Family Medicine, Geriatrics and psychiatry preferred.
  • Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations.
  • Board Certification in Palliative Care Preferred.
  • Active DEA license.
  • Able and willing to travel up to 50% as needed to support business and patient needs.
