Job description

Case Manager

The Case Manager will provide ongoing support and expertise to help formerly incarcerated and homeless women obtain knowledge, skills and access supportive services that provide the framework for understanding the problems they are experiencing while acquiring the skills that are necessary to lead them on a path to career obtainment and asset development. Case Manager reports directly to the Program Manager.

Responsibilities

1. Provide comprehensive case management services, including intake and needs assessments, benefit screening, service planning, goal setting, and ongoing progress monitoring.

2. Conduct outreach, recruitment, and enrollment activities to connect eligible individuals with programs and supportive services funded through government and grant-supported initiatives.

3. Assist clients in accessing public benefits, housing resources, workforce development opportunities, behavioral health services, and other community-based supports.

4. Develop individualized service, housing, and transitional action plans in collaboration with clients to support stability, recovery, self-sufficiency, and long-term goal attainment.

5. Maintain regular client contact to monitor participation, coordinate services, advocate with external agencies, and support progress toward established goals.

6. Coordinate referrals, appointments, and partnerships with community organizations and service providers to expand client access to supportive resources and training opportunities.

7. Support clients in recovery and behavioral change efforts by promoting healthy coping strategies, relapse prevention, treatment engagement, and participation in recovery support programs.

8. Maintain accurate, timely, and confidential case records, including service plans, progress notes, referrals, and follow-up documentation in accordance with agency policies and program requirements.

9. Identify barriers, risks, or service gaps impacting client success and recommend appropriate interventions or corrective actions.

10. Contribute to program effectiveness by participating in training, team initiatives, special projects, and other related duties as assigned.

11. Other duties as assigned.

Qualifications

12. Computer Literate in the following: Microsoft Word, Excel, Outlook, and emails. Must be able to demonstrate proficiency.

13. State and Federal laws and regulations on confidentiality, HIPPA, and other government standards.

14. Community resources and counseling/social work practices with high-risk populations and working with people in crisis.

15. Excellent written, verbal communication and documentation skills,

16. Ability to establish rapport and motivate others towards achieving goals.

17. Ability to work independently with strong sense of focus, task-oriented, and be nonjudgmental.

18. A strong sense of and respect for confidentiality involving both clients and fellow employees.

19. Ability to work in a variety of settings with culturally diverse families and communities with the ability to be culturally sensitive and appropriate.

20. Tolerant of stress.

21. The ability to recognize your own limitations and respond to difficult situations.

22. The ability to apply your knowledge and research to clinical problems.

1. Bachelor’s degree in social work or related field

2. Three years minimum of prior experience and knowledge of individuals recovering from the effects of incarceration, addiction and mental illness
