Job description

RiseWell Community Services is seeking a Full-time Companion Advocate to join the Senior/Junior Companion Program at Patchogue. Position is a Full-Time paid position (Mon-Fri 9AM-5PM).

In this role, you will be responsible for the daily operation of the Junior Senior Companion Program, including coordinating and directly supervising volunteer activities, providing volunteer training and orientation, completing program documentation, and data collection/reporting to comply with funding source requirements, in conjunction with the program supervisor.

Our Outreach programs provide an array of services designed to engage individuals who are reluctant to seek help or experience barriers to access. Services are brought to the person where they are located, whether that be at home, the street, a hospital, or another location. As a part of this team, you’ll assist those we serve to have their basic needs met by providing referrals, linkage, and hands-on assistance with obtaining food, employment, housing, and financial assistance.

QUALIFICATIONS:
Clearance of the following: Nationwide name-based NSOPW check. Statewide Criminal History Registry Check. Statewide Central Register of Child Abuse and Maltreatment. NYS Office of Mental Health Fingerprint-based FBI check (Fieldprint). Clean and valid NYS driver’s license
