We are currently seeking a APP (FNP, NP, or Adult Gero NP) or PA  based in Sullivan, IL, serving local skilled nursing facilities within a close-knit, mission-driven care community.

Full-Time APP Compensation and Benefits Package includes:

  • The estimated compensation range for this position is $104,000 to $169,000 per year, based on expected earnings under our uncapped productivity-based compensation model. This role is eligible for a longevity bonus of up to $50,000 and additional quality bonus programs.

What You’ll Do

  • Provide high-quality, patient-centered care to residents in skilled nursing facilities
  • Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
  • Communicate with facility nursing staff via phone or secure messaging as needed
  • Maintain reliable on-site attendance for scheduled shifts
  • Ensure compliance with clinical standards, facility policies, and regulatory requirements

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Employee must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation. Theoria Medical conducts criminal background checks and pre-employment drug testing on all candidates upon acceptance of contingent offer.
