Key Responsibilities

Provide support to the systems that deliver resources and services to patients.

Maintain appropriate documentation in State Database to reflect services rendered to clients under Ryan White Part B Funding.

Provide timely payment of medical bills, transportation costs, food-based services, emergency financial spending, and housing-related expenses as appropriate, ensuring all payments are documented accurately.

Assist with housing-related services for clients, including coordinating housing payments, supporting housing stability efforts, and helping facilitate housing inspections when required.

Assist clients with obtaining timely and coordinated access to medically appropriate levels of health and support services and continuity of care.


QUALIFICATIONS

Must have a Bachelor's Degree in case management or related field, or equivalent experience.

S/he should have a basic knowledge of medical case management principles.

Experience working in a nonprofit organization or a strong desire to work in service driven environment.

Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel

Valid Driver's License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
