The At-Home with Oaknoll Care Coordinator does more than just assist with managing the At-Home with Oaknoll program! Care Coordinators at Oaknoll help both staff and residents in the program feel that their needs are being met through empathetic service and communication, helping to CREATE home for our residents, building lasting relationships with them and other staff members. Oaknoll is a not-for-profit community that provides resident-centered care and services through our CREATE culture.

Oaknoll is currently seeking a full-time At-Home with Oaknoll Care Coordinator. While the exact schedule may vary according to the needs of our residents, a 40-hour work week is expected. Job responsibilities will include, but are not limited to: assisting with scheduling and billing using Aaniie software, training new caregivers, assisting with client care plans, keeping client and employee information up to date within Aaniie, filling open caregiving shifts as needed, responding to At-Home with Oaknoll emails and phone calls, and assisting the manager to make sure program needs are met.

Requirements
  • High school diploma or equivalent.
  • CPR Certification or the ability to be certified within 60 days of employment.
  • Certified as a Nurse Aide and active/eligible on the Iowa Nurse Aide Registry or the ability to be certified withing the first 60 days of employment.
  • Effective verbal and written communication skills in English.
  • Valid Iowa Driver's License and a clean certified driving record (provided before hire).
  • Adequate auto insurance.
  • Strong interest in serving the geriatric population.
