• Conduct thorough assessments for clients and their families to accurately identify their social, emotional, and psychological needs related to end-of-life care.
• Provide compassionate individual and group counseling employing evidence-based therapeutic methods tailored to the hospice context.
• Develop and execute comprehensive service plans that align with clients' goals, ensuring that each plan addresses their unique needs and circumstances.
• Serve as an advocate for clients, helping them navigate community resources, healthcare systems, and social justice issues that impact their lives.
• Engage in the development, evaluation, and reporting of programs to enhance the effectiveness of our social services.
• Collaborate closely with a diverse network of community organizations, government agencies, and volunteers to enrich client support services.
• Actively participate in team meetings, professional training, and community outreach initiatives to promote awareness and engagement.

The primary client population consists of individuals and families facing terminal illnesses and end-of-life challenges. Our work is centered on ensuring that clients experience dignity, support, and guidance during their most vulnerable moments. You will play a crucial role in providing emotional aid, counseling, and advocacy to enhance their well-being and assist with transition processes.

• Master's degree in Social Work (MSW) from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong foundational knowledge of social justice principles, advocacy strategies, and trauma-informed care practices.
• Exceptional communication skills coupled with a high degree of empathy and sensitivity to client needs.
• Solid organizational abilities conducive to managing multiple cases and responsibilities effectively.

• Prior experience working within a non-profit organization or healthcare setting is highly desirable.
• Familiarity with hospice care and palliative services will be considered an asset.
• Experience in developing educational programs or community outreach related to mental health and social support is advantageous.
