POSITION OVERVIEW

• Base salary up to $267,624 annually
• Additional bonus potential up to $44,000 annually
• Access bonus paid biweekly for panel target achievement

COMPENSATION & BENEFITS
• Annual base salary: $234,635.20 – $267,624.00
• $12,400 annual access bonus paid biweekly for panel target achievement
• Up to $44,000 additional annual bonus potential for higher patient volumes
• Partnership eligible sign-on structure:
– $50,000 sign-on bonus
– $25,000 retention bonus in year 2
– $25,000 retention bonus in year 3
• Non-partnership sign-on structure available
• $10,000 relocation reimbursement
• Additional $10,000 relocation assistance for parent or in-law relocation
• 29 days paid time off including CME, holidays, and sick time
• Comprehensive medical, dental, and vision insurance
• $5,000 annual company credit card for licensing, DEA, CME, and equipment
• Laptop and cell phone provided
• Language differential pay available for Spanish-speaking providers
• NHSC and federal loan repayment eligibility ranging from $75,000–$300,000
ROLE RESPONSIBILITIES
• Provide outpatient-only primary care services to diverse patient populations
• Manage acute and chronic primary care conditions within collaborative care model
• Partner with multidisciplinary support teams including 2 MAs and RN support per provider
• Utilize integrated services including behavioral health, telepsychiatry, chiropractic, acupuncture, and specialty consult resources
• Participate in quality improvement initiatives and team-based care meetings
• Deliver compassionate, mission-focused care to underserved rural communities

Requirements:
